Team Support Healthcare are working on behalf of a client based in Shrewsbury to provide recovery focussed support to individuals with complex and enduring Mental Health needs.
About you:

Experience of working with customers with complex mental health needs including – self harm, personality disorder and schizophrenia or personal experience

Self-motivated and able to work on own initiative

Experience of professional partnership-working

Deliver customer focused support in line with customer hours and service rotas

Ability to create and maintain personalised support plans

Assess risks,, write assessments and draw up risk management plans
-
To regularly hold key worker meetings to review and update customer outcomes
-
Ability to use Microsoft IT programmes

NVQ Level 2 in Health and Social Care or working towards

Experience in administering medication
